We ensure proper claim reporting takes place immediately after an incident and we handle the claim from start to finish.

Claims can be reported 24/7 by email. Should an incident ever occur, regardless of whether or not the employee wishes to seek medical treatment, we require an Incident Report be filled out. Our Incident Report can be found by clicking here.

If the employee does not wish to seek medical treatment, please have the employee complete the Refusal of Medical Treatment form that can be found below.
